We put you at the heart of everything we do at Outcome Care and Support.

We work in partnership with the people we support as we believe that to really ensure person centred care and support, we need to find out what matters to you and design a holistic care plan that meets your own defined needs and outcomes.

Our ethos is to enable you or your loved one to do as much as they can for themselves to ensure that you are able to uphold your quality of life and that you are able to live at home for as long as possible. Our team will strive to uphold your rights, independence, choice and inclusion when it comes to planning your support, involving you every step of the way. We strengthen this approach through involving your family and friends if you choose to do so.

Our Homecare, Live in and Community Nursing teams are led by our two Directors who are both experienced Registered Nurses. Our staff are trained health care professionals and Registered nurses who deliver our services with our ethos of enablement, ensuring that it is at the forefront of everything that they do.
